Alternate Solutions Health Network Licensed Practical Nurse (LPN) - Hospice in Troy, Michigan
Beaumont Home Health and Hospice is a fast growing hospice network looking for an Licensed Practical Nurse (LPN), Hospice to join our team!
As a LPN, Hospice you will provide comprehensive skilled care as ordered by the attending physician by following the order sets created by the RNCM. The LPN educates patients and their family members and ensures the safety of the patient. The LPN is accountable for completing accurate documentation and remains compliant with all legal rules and regulations.

Provide skilled nursing care in a patient’s home as ordered by the attending physician
Perform care by following order sets and goals made by RN
Observe and monitors patient conditions
Notify RNCM of any changes in patient condition
Reconcile medications prescribed by the physician
Counsel patient and family on the disease/progression and how to manage
Incorporate patient and family in development of plan of care
Listen to patient and family members
Completes all clinical documentation in accordance with agency protocol and Medicare/Federal guidelines
Documents all aspects of treatment, assessments, and patient education
Other duties as assigned
Licensed Practical Nurse with current license in the state of MI
One year of experience as an LPN in an acute care setting is strongly recommended
Hospice care experience is desired
Ability to create positive impressions and communicate with a variety of people
Valid driver's license and auto insurance
