Beaumont Home Health and Hospice is looking for an organized and driven professional to join us as a General Inpatient Nurse - Case Manager for Hospice.
As a General Inpatient Nurse - Case Manager for Hospice, you will be responsible for end-of-life care and Hospice services to patients in the Beaumont Inpatient Hospice setting in a manner consistent with Beaumont Hospice standards of practice. The Hospice General Inpatient Nurse will deliver care in a holistic, family-centered manner to help ensure their comfort and quality of life while under their care. You will also provide patient healthcare coordination services and may attend discharge/multidisciplinary rounds in acute care, ambulatory and or other settings within the health system to share expertise and to assist the patient in transition of care from one setting to the next within the health system.

Plan and deliver care to patients utilizing the nursing process of assessment, planning, interventions, implementation, and evaluation
Effectively interact and support patients, families and other interdisciplinary team members while maintaining standards of professional nursing.
Maintain the dignity of the dying patient while supporting their unique spiritual and cultural beliefs.
Provide support to patient’s family/caregivers at end-of-life and time of death.
Attend interdisciplinary team meetings for team discussions, consultation, evaluation of patient/family needs and appropriateness of care.
Maintain documentation for continuity of care, including direct patient care, medication administration, phone calls, communication, visits, and other pertinent clinical information.
Participate in Quality Assessment Performance Improvement (QAPI) activities as requested
Coordinates patient care for optimal transition from the hospital to home setting where hospice services will be provided
Completes all clinical documentation in accordance with agency policy and Medicare/Federal guidelines
Attends all mandatory meetings
Other duties as assigned
RN—Graduate of Diploma or Associate Degree program in Nursing; Bachelor’s Degree in Nursing preferred
Active RN license in the state of employment, without any board actions
Minimum 3 years acute care experience. Hospice experience preferred
Computer skills sufficient to properly document services and care provided
Strong written and verbal communication skills
Critical-thinking skills needed to assess a patient’s situation, as well as detect changes in symptoms, health or pain, and act as necessary
Organizational skills with the ability to prioritize and reprioritize in a rapidly changing day-to day environment
