Munson Healthcare Coordinator Quality Incentives in Traverse City, Michigan

ENTRY REQUIREMENTS

Must be a LPN, Certified Medical Assistant or have an Associate’s degree in a health care discipline.

A proven high level of health provider relation skills combined with excellent oral and written communication skills are essential.

Must be self-motivated and have the ability to work independently as well as a team player to accomplish goals.

An accomplished level of independent analytical ability required to compile, analyze and interpret data.

ORGANIZATION

Reports to the Manager of Family Practice

SPECIFIC DUTIES

  • Supports the Mission, Vision and Values of Munson Healthcare

  • Embraces and supports the Performance Improvement philosophy of Munson Healthcare.

  • Promotes personal and patient safety.

  • Has basic understanding of Relationship-Based Care (RBC) principles, meets expectations outlined in Commitment To My Co-workers, and supports RBC unit action plans.

  • Uses effective customer service/interpersonal skills at all times.

  • Optimizes quality improvement opportunities to enhance incentive payments and avoid penalties from insurers.

  • Coordinates Patient Centered Medical Home (PCMH) incentives to improve care and maximize incentives by

  • Maintaining documentation manual for PCMH domains

  • Tracking and follow up for referrals, labs, and diagnostics

  • Conducting internal evaluation of listed PCMH capabilities

  • Identifying additional PCMH capabilities and workflow needs

  • Staying current with PCMH domains and functions.

  • Supports continued fulfillment of Meaningful Use requirements by

  • Documenting transfer for transitions of care, including electronic transfer

  • Utilizing NextGen Health Quality Module for Clinical Quality Measure

  • Sending out preventive care reminders

  • Electronic importing of Continuity Care Document data

  • Coordinate activities to maximize incentives from insurance Pay For Performance by

  • Report and enter data to insurance companies websites.

  • Patient recall reminders

  • Identify opportunities to enhance Pay For Performance

  • Lead meetings with management on pay for performance updates

  • Coordinate Other Quality Improvement Activities

  • Generate practice and provider quality reports utilizing NG HQM

  • Provide staff support for NextGen Dashboard reporting

  • Provide staff support for Quality Improvement projects

  • Manage recalls for patients overdue for chronic disease management

  • Educates providers and residents on Patient Centered Medical Home, Insurance incentives, and Meaniful Use requirements.

  • Additional duties may include supporting or providing back up for check in/check out, medical records, phone triage, Clinic schedules, and aspects of direct patient care (depending on clinical training).
