Munson Medical Center Coordinator- Post Acute Care in Traverse City, Michigan

SUMMARY The Post-Acute Coordinator (PAC) is accountable for facilitating, implementing, and confirming post-acute transition plans for the patient population. The position will respond to requests for post-acute services from inpatient clinicians and community-based providers.

ENTRY REQUIREMENTS Social work technician license, Associate’s degree in a healthcare discipline preferred or an equivalent combination of education and experience totaling two years of hospital/medical office experience required.

Experience with the patient discharge process and post-hospitalization placement is preferred

Experience in supporting professional staff activities may be substituted.

Effective interpersonal communication skills is expected. Must be able to demonstrate the ability to prioritize tasks and supervise support staff.

Knowledge of medical terminology required.

ORGANIZATION Reports to the Manager of Social Work - Case Management.


SPECIFIC DUTIES 1. Supports the Mission, Vision and Values of Munson Healthcare

  5. Partners with the Case Manager, Social Worker, Nurse or other clinical associates to prioritize and implement post-acute placement in a SNF, rehab, sub-acute, assisted living facility or other venue as ordered.

  6. Facilitates prompt processing of all requests for post-acute services including durable medical equipment (DME), and appointments with community resources.

  7. Confirms patients’ benefits, identify preferred providers and use knowledge of community availability to effect timely disposition.

  8. Identifies availability, completes and transmits necessary forms to implement and complete the discharge and placement process. Uses electronic software to distribute post-acute placement requests to community and regional resources and.

  9. Establishes and maintains routine communication with referring Case Managers, Nurses, Social Workers, Hospitalists or other prescribing member to keep them informed of the progress of their request.

  10. Identifies obstacles to timely transition and collaborate with the Case Manager, Nurse, or Social Worker to resolve.

  11. In collaboration with requesting clinician, arrange safe transportation that best fits the patient’s needs and financial resources.

  12. Updates all involved parties regarding progress, revisions and other information related to implementing the plan.

  13. Maintains log of any quality issues encountered with preferred providers that require follow-up.

  14. Resolves any pre-certification, registration or form completion prior to setting up an appointment with a community provider.

  15. May participate in follow-up processes to discharged patients to query about quality and timeliness of post-acute arrangements.

  16. Demonstrates independent problem solving skills on behalf of the Care Management team and the patient.

  17. Professionally respond to payer’s request for information.

  18. Demonstrates effective time management skills.

  19. Demonstrates knowledge of and adherence to departmental policies.

  20. Promotes program stability, reputation, and growth by coordinating work with colleagues.

  21. Appropriately receives constructive guidance from senior staff and program managers.

  26. Demonstrates knowledge of current regulatory and accrediting agency requirements as it pertains to obtaining services for patients.

  27. Attends and participates in team building activities, daily huddles, patient care conferences, and monthly departmental meetings.

  28. If social work technician license or Bachelor’s in Social work license (must be supervised by a Master’s prepared Social Worker or RN/MSW Patient Care Manager), may also perform these duties:

a. Interviewing clients to obtain information about the client’s situation and to provide info about service and community resources

b. Conducting case-finding

c. Monitoring client’s compliance with a program

d. Provide life skills training
