Munson Healthcare Clinical Nurse Practitioner in Traverse City, Michigan


To assist the nephrologists in the provision of episodic and continuity of care to ESRD and CRD patients who are patients of Nephrology Consultants and admitted to Munson Medical Center and dialysis centers affiliated with Munson Medical Center.


A Master’s degree in Nursing, with current State of Michigan licensure both as a Nurse and as a Nurse Practitioner.

Candidate must apply for and be granted appropriate clinical privileges through MMC’s normal credentialing processes before rendering services

Renal experience preferred.

Must possess a high degree of interpersonal skills, with experience in working with multi-disciplinary medical and health care teams.

Experience in providing inpatient continuity of care and outpatient chronic care.

Possesses current Michigan motor vehicle license, ability to drive a vehicle in all weather conditions, and capable traveling to other dialysis locations.

Computer literacy.


The position is under the employ of Munson Medical Group. Receives general supervision from the Medical Director for Nephrology Services. The incumbent will be expected to work effectively and positively with Medical Staff Physicians. Additional responsibilities may be assigned by Administration.

No direct clinical contact with patients

Working closely with the Nephrologists and Medical Center Medical Staff, provides effective and efficient care for both episodic and continuity patients in the dialysis programs and hospital inpatient. This will include the initial evaluation of patients and providing, under the direction of the Nephrologists, ongoing care for patients to which they are assigned.

  5. Responsibility for episodic and longitudinal care of patients who are admitted to the dialysis centers and to Munson Medical Center.

  6. History and physical assessment, performing patient rounds, patient follow-up, family support, and other tasks as designated by the Nephrologists.

  7. Coordinates referrals for other medical services.

  8. Performs other duties and responsibilities as assigned.
