Discusses established discharge goals during treatment and provides information on goals met/unmet to physical therapist./bbParticipates in educating patient and family/caregiver per prescribed physician's plan of care. Instructs patients in care and use of equipment as directed by physician and physical therapist./bbDisplays high regard for personal dignity, diversity and uniqueness of individuals (Cultural/Religious Sensitivity; Confidentiality; Privacy; Age Specific Competencies)./bbEvaluate patient needs on a continuous basis recognizing changes that need to be made in the plan of care and notifying the Physical Therapist of these needs. Makes joint visits with the Physical Therapist as needed./bbMaintains interdisciplinary communication with other disciplines via written and verbal means./bbComplete team meeting reports including activities, patient's response to treatment and progress toward goals./bbCoordinates scheduling of patients/bbCompletes required agency forms to record time and patient progress in a timely manner. Assists with other clinical record keeping as deemed appropriate./bbParticipates in inservice programs and staff meetings as required./bbMaintains confidentiality with all information in the appropriate manner./bbContributes to organization success targets for patient satisfaction and net operating margin by providing excellent customer service performance./bband#160;/bbOther information:br/bbEDUCATION/EXPERIENCE/bbGraduate of a two-year college level program approved by the American Physical Therapy Association./bbAt least one year of experience as a Physical Therapist Assistant/bbPrevious homecare experience preferred/bbBLS certified or eligible/bband#160;/bbKNOWLEDGE/SKILLS/ABILITIES/bbHomecare knowledge preferred/bbAbility to type 25 WPM/bbDemonstrate critical thinking skills/bbComfortable with basic computer programs and e-mail/bbStrong interpersonal skills/bbGood organizational skills/bbSelf-motivated/bband#160;/bbWORKING CONDITIONS/PHYSICAL DEMANDS/bbAbility to maintain punctual attendance consistent with the ADA, FMLA, and other federal, s
