Description#160;#160;#160;#160;#160; COVENANT HEALTHCARE#160;#160;#160;#160; US:MI:SAGINAW#160;#160;#160;#160; 8:00 AM - 4:30 PM, MONDAY - FRIDAY#160;#160;#160;#160; FULL TIME BENEFITED#160;#160;#160;#160; Summary:#160;#160;#160;#160;#160;#160;#160;#160;#160; Demonstrates excellent customer service performance in that his/her attitude and actions are, at all times consistent with the standards contained in the Vision, Mission and Values of Covenant HealthCare and the commitment to Extraordinary Care for Every Generation.#160;#160;#160;#160;#160;#160;#160;#160;#160; The PHO is recruiting a Clinical Pharmacist Specialist in Ambulatory to improve patient care outcomes by reviewing prescribing patterns, managing policies, protocols, and guidelines designed to improve the safe, effective, and cost-effective use of medications, teaching healthcare professionals on all aspects of medication use policy, providing health care professional education, and conducting medication use evaluations to ensure compliance with restrictions and other policies.#160;#160;#160;#160; Responsibilities:#160;#160;#160;#160;#160;#160;#160;#160;#160; * As an integral member of the Physician Hospital Organization, this individual will work closely with physicians, medical staff, nursing, and committees to develop and implement medication use policies which ensure safe and effective medication use. In this role, they will interface with medical specialties regarding medication formulary, medication use standards, medication improvement initiatives, and evidence-based use of medications. The selected individual will:#160;#160;#160;#160;#160;#160;#160;#160;#160; * Assists physicians in disease management within institution approved collaborative practice agreements (diabetes, hypertension, hyperlipidemia, asthma, COPD, anticoagulation, others), referral-based Comprehensive Medication Review with recommendations to physicians, daily ad hoc informal consultations to improve pharmacotherapy effectiveness, safety, and cost as well as participating in team based care within a multidisciplinary model.#160;#160;#160;#160;#160;#160;#160;#160;#160; * Educate health care professionals using multiple communication methods, such as presentations, newsletters, emails, the pharmacy intranet, one-on-one sessions, and other means#160;#160;#160;#160;#160;#160;#160;#160;#160; * Provides education and information on pharmaceutical care and drug therapy to patients, pharmacy staff, physicians, nurses, students and other clinicians.#160;#160;#160;#160;#160;#160;#160;#160;#160; * Provides drug therapy management and assessment including identifying and resolving potential and actual drug related problems.#160;#160;#160;#160;#160;#160;#160;#160;#160; * Provide clinical recommendations pertaining to gaps in care, high risk medications, compliance and adherence, drug interactions, therapeutic substitution#160;#160;#160;#160;#160;#160;#160;#160;#160; * Utilize formulary information to guide appropriate medication recommendations#160;#160;#160;#160;#160;#160;#160;#160;#160; * Keep current with new and emerging evidence-based medicine and therapeutic recommendations
