Memorial Health Care Center PCT in Owosso, Michigan


The PCT works under the supervision and/or direction of the Meadows Administrator, clinical nurse manager, and supervisor or team leader. Performs duties and assists staff nurses in the treatment and care of residents in accordance with the established methods and procedures of the Meadows assisted living facility. Strives for superior performance and consistently provides a product or service to leadership and staff, which is recognized as ultimately contributing to the resident and family experience. Recognizes and demonstrates understanding of resident and family centered care.


  • Assists the nurse with resident admissions,

  • Provides personal care (hygiene, ADL's, ambulation, etc.) as assigned.

  • Performs routine treatments and specimen collections, according to operating instructions. Reports and documents as appropriate any changes observed in the condition or behavior of a resident (Includes vital signs, I & O, blood glucose, and daily weights).

  • Assists residents at meal time and with nourishments.

  • Transports residents to and from various areas, as necessary.

  • Makes regular rounds on assigned residents and responds to resident call lights as needed.

  • Assists in maintaining a safe environment by cleaning equipment and reporting equipment that is out of order, preparing equipment for disinfection, and maintaining stock supplies on the units as directed.

  • Attends in-service education programs as assigned. Complete all new learning as assigned.

  • Documents resident interventions in computerized charting or on paper.

  • Demonstrates knowledge of and supports hospital mission, vision, value statements, standards, policies and procedures, operating instructions, confidentiality statements, corporate compliance plan, customer service standards, and the code of ethical behavior.

  • Must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of residents served on his/her assigned unit.

  • Assists with resident activity programs on weekends, evenings and as needed.

  • Other duties as assigned.



  • Requires level of education necessary to read and understand orders, to chart vital signs, and other information on resident's chart.

  • Ability to perform simple math functions.

  • Nurse Aide certification required.

  • Certification in Healthcare Provider BLS required.


  • Previous experience as a patient care technician preferred.


  • Access a resident in bed and his/her equipment for the purpose of obtaining vital signs and performing ADL’s.

  • Safely move residents as ordered.


  • Proficiency using modern office, computer and telephone equipment as used by Memorial Healthcare.

  • Have hand-eye coordination and fine motor skills for administration of treatments (i.e. weights, hot packs, enemas, TED stockings, vital signs, etc).


  • Ability to adapt and maintain focus in fast paced, quickly changing or stressful situations.

  • Ability to read and interpret a variety of documents including, but not limited to policies, operating instructions, white papers, regulations, rules and laws.

  • Identify changes in a resident’s condition and/or resident’s response to interventions.

  • Document findings and interventions as appropriate.


  • Able to see for the purpose of reading information received in formats including but not limited to paper, computer, reports, bulletins, updates, manuals.

  • Able to see and hear for work-related purposes.

  • Able to communicate effectively with residents and family.

  • Visually assess resident’s condition.

  • Auscultate blood pressures.


  • Ability to interact with co-workers, hospital staff, administration, residents, physicians, the public and all internal and external customers in a professional and effective, courteous and tactful manner, at all times, physically, verbally and in all written and electronic communication.

  • Required to remain calm when adversity is encountered.
