This RN Program Alignment Coordinator role will be focusing with the Geriatric Office Location at Mercy Health Physician Partners covering Grand Rapids.

POSITION SUMMARY

Consults with Providers for the coordination tracking and reporting of quality management and process improvement for assigned regional practices within Affinia Health Network using facilitation, collaboration, medication and change management as key tools to accomplish program alignment. Works under the direction of the Affinia Health Medical Director or their designee, with dotted lines of responsibility to the Employed Physician Network leadership in the local Affinia Health Networks.

POSITION QUALIFICATIONS

Education

Minimum: Bachelor's Degree, preferably in Nursing or Quality Assurance.

Preferred: Master's Degree in Nursing

Credentials/ Licensure

Minimum: Current State of Michigan Registered Nursing License. CPR or BCLS Certified.

Preferred: Certified Nurse in sub specialty (OCN, CNRN) and other sub specialty (Quality) Certification.

Related Experience

Minimum: Experience in administration and physician office practice.

Preferred: Two years of experience in quality management and payor incentive program.

Computer Competency

Familiarity with standard desktop and windows based computer system, including email, e-learning, intranet and computer navigation. Ability to use other software required to perform essential functions.

Physical/Mental

Minimum: Independently mobile; computer work/keyboarding; ability to transport objects weighing up to 20 pounds; ability to handle multiple conflicting demands.

ESSENTIAL FUNCTIONS

 Builds consensus and alignment for process improvement initiatives using CQI/Lean methodologies and principles.

 Coordinates and implements quality management and improvement processes within the assigned Regional Programs.

 Facilitates communication between assigned Regional physician practices to ensure alignment of clinical process to meet patient needs and program requirements.

 Coordinates, aligns and tracks the implementation of quality assessment and improve¬ment activities.

 Facilitates and builds consensus for the development and implementation of processes intended to improve service, utilization & quality.

 Facilitates quality/utilization/program education for the practices.

 Oversees the implementation and maintenance of electronic tracking tools for the monitoring of program outcomes and coordination of the submission of reporting to meet program requirements to payers, practices and administration.

 Consults with others to assure patients receive a seamless continuum of care across the health system.

 Acts as a liaison between health plans and the program by attending and coordinating meetings and communications.

 Attends staff meetings to facilitate communication and build staff relationships.
