This position will be responsible for coordinating the creation and use of a 'complex care map' that documents the recommended management of all aspects of patient-centered care for an identified panel of complex care patients. The Complex Care Coordinator interfaces with physicians, health care teams, patients and their caregivers to assess the patient's needs and detail the plan of care as recommended by the interdisciplinary team members. In conjunction with a physician dyad partner, the Complex Care Coordinator ensures the care map is submitted in a timely manner to the Review Board, and once approved, is uploaded into the electronic medical record system. Additionally, this position is responsible for assisting the interdisciplinary team in adhering to established care maps and acting as a consultant where needed.
REQUIRED EDUCATION, EXPERIENCE AND LICENSURE:
BSN/Bachelors in Science in Nursing
Current State of Michigan RN licensure
Minimum of two (2) years of nursing. Case Management, Community Health or Care Coordination experience is strongly preferred.
