Henry Ford Health System Full-Time (NP/PA/MASTER’S RN) – NON-CLINICAL Post-Acute / Skilled Nursing Facility (JACKSON, MI) in Jackson, Michigan


A non-clinical position, where provider is responsible for liaison and community outreach lead. Provider participates as a leader of the skilled nursing facility (SNF) care team. Visits designated facilities, provides appropriate education, coordinates patient needs through continuity of care.

Minimal clinical requirements for APP candidates seeking opportunity to be a minimum of 200 hours per year which equates to approx. 10% of scheduled hours. Clinical area assigned is determined by the candidate's clinical background within the HFAH organization. For external candidates, or those without clinical experience, the clinical area assigned will likely be Hospice and Advanced Illness Management services within the HFA at Home department

In this administrative role, provider’s responsibilities include but are not limited to improving quality and decreasing admissions. Using advocacy for education and problem solving.

Designs, plans and implements a transformational approach to systematic to care coordination. Responsible for leading, directing and fully deploying innovating initiatives that will transform models of care, which support the individualized needs of patients. Utilizing evidence-based approaches to improve care delivery, promote coordination of care, and improve outcomes. Operates under minimal direction and reports to executive leadership.

  • Position is Monday – Friday, 8a5p

  • No Call; but provider should be available to answer questions from the Central Market on an as needed basis

TRAVEL EXPECTATIONS: With the COVID surge, many responsibilities are virtually. Once the pandemic is under control, on average, provider can expect 30-60% travel on a weekly basis, with the understanding that expansion is in the future. Mileage is reimbursed, based on the federal rate.

WORKING REMOTELY: All equipment is supplied for provider to do their job successfully. Provider must be willing to adapt and adjust to remote work. We do not see this changing in the future; however, a physical office is designated at our One Jackson Square location; 100 E Michigan Ave Jackson, MI 49201

Unpredictable environment with wide variations in the types of patients, acuity levels, times of admissions/discharges and the number of patients/health care team members works in a standard patient care area with varying conditions regarding temperature control and potential exposure to hazardous materials.


  • Master’s RN Licensure OR Nurse Practitioner Certification/Physician Assistant Licensure, required (in the State of Michigan)

  • Valid Driver’s License, with good driving record, vehicle in good working condition and vehicle insurance, required

  • Knowledge of SNF standards, concepts, and regulations, required

  • SNF Experience, preferred

  • Ability to travel to perform rounding, daily
