Holland Hospital Home Health Supervising Clinical Coordinator -Certified in Holland, Michigan

The Supervising Clinical Coordinator assists the Manager in overseeing the day-to-day operations of the unit. This function is in accordance with Michigan Public Health Code Act 368 of 1078, Article 15 (Health Occupations), and Part 172 (Nursing). Under the direction and delegation of the Manager, assists with planning, implementing and evaluating current practices to promote high-quality, cost-effective care; ensures focus and emphasis on enhancing patient and staff satisfaction; performs other duties as assigned. This position must be able to demonstrate the knowledge and skills necessary to provide care and service appropriate to the age of the patients served in his/her area.

Minimum Education/Training: 2 or more years clinical experience in Home Health. Graduate of an approved school of nursing (BSN required or obtained within 5 years of hire)

Registration/Certification or License Required: Licensed to practice professional nursing in the State of Michigan.

Preferred Experience: 3-5 years of Home Care experience. Supervisory or Administrative experience preferred.

Physical Requirements: Lift and carry 40-40 lbs. Push/pull +/- 150-200 lbs. (Patients on bed, stretcher). Lateral transfer 150-200 lbs. (e.g. Patient). Occasional standing and walking. Occasional twisting, bending and squatting. Occasional reaching. Frequent keyboard use. Continuous sitting. Continuous talking or hearing.

Other Requirements: Exceptionally dependable, emotionally stable, and have ability to work under stress. Excellent written and verbal communication skills and conflict resolution skills. Ability to work efficiently, effectively and independently under deadlines and high volumes. Highly organized, self-directed, and detail oriented with the ability to prioritize. Energetic and able to learn quickly and adapt to change quickly. Ability to cover weekend on call hours on a rotating basis. Computer literate. Proficient with Microsoft Office. Reliable means of transportation. Valid driver's license and automobile insurance.
