Holland Hospital Diabetes Care & Education Specialist in Holland, Michigan
Provides individualized diabetes self-management education/training to individuals and groups according to the Scope of Practice, Standards of Practice, and Standards of Professional Performance for Diabetes Educators.
This Diabetes Care & Education Specialist will work as a diabetes care manager within a physician practice working alongside providers, nurse care managers & behavioral health specialists in the care of our diabetes population.
Minimum Educator/Training: Bachelors of Science in Nursing (BSN) or Health related field. Credentialed as a Certified Diabetes Care and Education Specialist or completed within one year of hire. RN or RD with minimum of 2-4 years recent experience in diabetes education. Experience in diabetes care and/or patient education.
Registration/Certification or License Required: Current RN in the State of Michigan or Cert Registered Dietitian. Current BLS for the Healthcare Provider or completed within 60 days of hire.
Physical Requirements: Lift and carry 25-35 lbs. Push/pull 50-100 lbs. (ie. empty bed, stretcher). Occasional standing, walking. Occasional twisting, bending, reaching, squatting, kneeling. Frequent sitting. Frequent keyboard use. Continuous talking, hearing.
Other Requirements: Ability to communicate verbally and nonverbally with honesty, openness, empathy and tact. Good listening skills. Ability to resolve conflicts constructively. Efficient in Microsoft Office. Reliable transportation for travel to worksite and community events.
