Our home health agency is an accredited agency that is hiring a home health registered nurse that is seeking a patient-centered care model of nursing. We work with patients of all ages to provide safe, quality care in a patient’s home within a 70 mile radius of Grand Rapids, MI. Registered nurses use their clinical judgment and assessments to coordinate the care of the patient with an interdisciplinary team.
Assess and educate patients each visit and document at point of care;
Perform evaluation tasks, including vitals and med review;
Administer and educate patient or caregiver on medications ordered by physician;
Coordinate care with all disciplines;
Ability to travel to multiple patients per day.
Education - Associate's Degree or equivalent in Nursing
Licenses - Registered Nurse (RN)* State of Michigan
Drivers License – MI (MI DR LIC)
Basic Life Support (BLS) within 90 Days
Education - Bachelor's Degree in Nursing
Experience - 1 year of experience typically gained through skills/knowledge/abilities in of acute care, emergency, or rehab facility experience for all of the Home Care business lines
