RN/LPN:
Our home health agency is a CARF accredited agency that is hiring a home health registered nurse that is seeking a patient-centered care model of rehab nursing. We work with patients of all ages to provide safe, quality care in a patient’s home within Grand Rapids, MI and surrounding areas. Our Nurses provide one to one care for rehab patients in the community setting. Nurses use their clinical judgment and assessments to coordinate the care of the patient with an interdisciplinary team.
Responsibilities:
Assess and educate patients each visit/shift and document at point of care
perform evaluation tasks, including vitals and med review
administer and educate patient or caregiver on medications ordered by physician
wound care
infusion therapy
-respiratory care

LPN Position Summary:
Under the direction of the RN / Mid-level Provider / Physician and in collaboration with other members of the health care team, contributes to the assessment, planning, implementation, and evaluation of patient care.

Delivers nursing care responsibly and accountably as described professionally by the American Nurses Association (ANA) and legally by the State of Michigan. Demonstrates competency through the critical thinking model known as the nursing process. This includes assessment, diagnosis, outcomes identification, planning, implementation, and evaluation. Upholds the standards of professional performance as described by the ANA. This includes ethical practice, culturally congruent practice, communication, collaboration, leadership, education, evidence-based practice and research, quality of practice, professional practice evaluation, resource utilization and environmental health. Strives to achieve optimal outcomes.

RN
Basic Qualifications:
Education - Associate's Degree or equivalent in graduate of an approved nursing program
Experience - Successful completion of a competency based orientation program
Licenses - Registered Nursing license in MI (RN)
Driver's License - MI - currently unrestricted, automobile insurance, and available automobile for NRS Home Care
Cardiac Pulmonary Resuscitation (CPR) within 180 Days of hire
Basic Life Support (BLS) within 90 days of hire
Advanced Cardiac Life Support (ACLS) within 1 year of hire
Preferred Qualifications:
2+ years homecare or med surg experience
Excellent time management skills
Compassionate
Ability to travel to multiple patients per day

LPN:
Basic Qualifications:
Education - High School Diploma or equivalent
Associate's Degree or equivalent -graduate of an approved nursing program
Successful completion of competency based orientation program
Licenses - Licensed Practical Nurse in MI (LPN) State of Michigan
Drivers License – MI (MI DR LIC) State of Michigan for NRS Home Care
Basic Life Support (BLS) within 90 Days of hire
Certified Brain Injury Specialist (CBIS) Certification for Continuing Care within 2 years
Preferred Qualifications:
Experience - Healthcare experience
2+ years homecare or med surg experience
Excellent time management skills
Compassionate
Ability to travel to various community locations
