Job Description

SummaryResponsible for accurate completion of the Minimum Data Set (MDS) and Care Area Assessments (CAAs) collaborating with Staff Nurses, Nurse Aides, and the interdisciplinary team on the assessment process and development of resident MDS/Care Plans. Position will monitor Medicare and Medicaid requirements.Qualifications

  • Associate's Degree equivalent related field Required

  • Bachelor's Degree equivalent related field Preferred

  • 2 years of relevant experience Nursing and MDS process with demonstrated nursing assessment skills Required

  • Experience in sub-acute care and knowledge of Prospective Payment System (PPS) Preferred

  • NURSING\Registered Nurse (RN) Required

  • CLINICAL\Nurse Assessment Coordinator, Registered (RNAC) Required

  • LIFE SUPPORT\Basic Life Support (BLS) 60 Days Required
