Position Purpose:

Provide care to residents related to activities of daily living and the resident Service Plan. Must possess a positive attitude and compassion for older adults.

Maintains a working knowledge of applicable Federal, State and local laws/regulations; the Trinity Health Integrity and Compliance Program and Code of Conduct; as well as other policies and procedures in order to ensure adherence in a manner that reflects honest, ethical and professional behavior.

Job Description Details:

Critical (highest priority; absolutely key to success on the job)

  • Assist residents with activities of daily living and ambulation, including bathing, dressing, eating, toileting, hygiene, supervision and mobility.

  • Maintain an awareness of each resident’s general state of physical and mental health and report any changes promptly to the Assisted Living Nurse Manager or nurse on duty.

  • Follow the individual resident’s Service Plan.

Important (secondary to critical responsibilities; contributes significantly to success on the job)

  • Maintain a clean, quiet, and safe environment for residents, families, visitors, and staff.

  • Answer calls for assistance and attend to resident needs promptly.

  • Inform nurses of “unusual incidents” or changes in resident status.

  • In collaboration with Assisted Living Nurse Manager, meet and help with orientation for new Assisted Living residents.

  • Oversee with meal services, including meal delivery, observing residents, transporting residents to dining room, nutrition and hydration.

  • Assist staff with tasks that require more than one person.

  • Promote resident’s feelings of self-worth, self-esteem, and independence.

  • Maintain records according to policy.

  • Assist with activity program as assigned.

  • Assist residents in keeping their apartments neat, clean and orderly.
