Expected Weekly Hours:
Provide care to residents related to activities of daily living and the resident Service Plan. Must possess a positive attitude and compassion for older adults.
Maintains a working knowledge of applicable Federal, State and local laws/regulations; the Trinity Health Integrity and Compliance Program and Code of Conduct; as well as other policies and procedures in order to ensure adherence in a manner that reflects honest, ethical and professional behavior.
Job Description Details:
Critical (highest priority; absolutely key to success on the job)
Assist residents with activities of daily living and ambulation, including bathing, dressing, eating, toileting, hygiene, supervision and mobility.
Maintain an awareness of each resident’s general state of physical and mental health and report any changes promptly to the Assisted Living Nurse Manager or nurse on duty.
Follow the individual resident’s Service Plan.
Important (secondary to critical responsibilities; contributes significantly to success on the job)
Maintain a clean, quiet, and safe environment for residents, families, visitors, and staff.
Answer calls for assistance and attend to resident needs promptly.
Inform nurses of “unusual incidents” or changes in resident status.
In collaboration with Assisted Living Nurse Manager, meet and help with orientation for new Assisted Living residents.
Oversee with meal services, including meal delivery, observing residents, transporting residents to dining room, nutrition and hydration.
Assist staff with tasks that require more than one person.
Promote resident’s feelings of self-worth, self-esteem, and independence.

Maintain records according to policy.
Assist with activity program as assigned.
Assist residents in keeping their apartments neat, clean and orderly.
