Evening Shift

Position Purpose:

Provide medication administration services to assigned residents in accordance with the Personal Care Physician directions and Home For the Aged regulations. Perform specific activities under the direction and supervision of the House Manager/Memory Care Manager. Must possess a positive attitude and compassion for older adults. Must have the desire and ability to work with seniors with memory impairment in a social model of care. Must be able to read, write, and speak English fluently.

Maintains a working knowledge of applicable Federal, State and local laws/regulations; the Trinity Health Integrity and Compliance Program and Code of Conduct; as well as other policies and procedures in order to ensure adherence in a manner that reflects honest, ethical and professional behavior

Job Description Details:

Critical (highest priority; absolutely key to success on the job)

  • Administer medications accurately while following the six “rights” of medication administration.

  • Observe resident to detect response to specified types of medications and prepares report or notifies designated personnel of unexpected reactions.

  • Maintain timely, accurate records in the Medical Administration Record (MAR) and resident’s chart

  • Follow accountability procedure for all controlled substances.

  • Respond to health emergency needs and follow appropriate protocol on care, notification, and documentation.

  • Provide personal care to the resident, as needed (i.e. assist with bathing, toileting, dressing) while facilitating the resident’s best ability to function and integrating Life Story preferences.

  • Report changes in resident condition (i.e. change in skin status, appetite, ADL ability, quality of life, activity participation, etc.) to Memory Care Manager and/or House Manager.

Important (secondary to critical responsibilities; contributes significantly to success on the job)

  • Promote residents feelings of independence, self-worth and self-esteem. Maintains dignity of resident in all instances.

  • Performs basic nursing aide duties.

  • Demonstrate kindness and respect toward all residents.

  • Assist staff with tasks that require more than one person.

  • Maintain med carts and ensure that they are clean, orderly and locked when not present.

  • Follow all standard precautions and infection control procedures when administering medication.

  • Maintain a clutter-free environment.


High school diploma or GED required.

Licensure / Certification

Graduate of an approved Medical Assistant Program or Medication Administration course.


1 year experience performing medication administration/distribution in a facility or residential group setting is preferred.
