div brGENERAL SUMMARY:Under the supervision of the Director of Nursing, the Admissions Coordinator facilitates admissions to BCHC by retrieving referrals, conducting pre-admission screenings, and by communicating with referral sources accordingly. This position requires excellent customer service and communication skills, as this individual will be the primary source of contact for referral sources throughout the community.span /spanMust project a positive attitude.ESSENTIAL DUTIES:span/spanspan /span1.span/spanObtain referrals via telephone, fax, or in person.span /span2.span /spanContact discharge planners to facilitate admissions or obtain additional information as needed.3.span /spanObtain needed information regarding BGH referrals from the CDR web system.4.span /spanObtain insurance verification and authorization for third party admissions.span /spanProvide information regarding reviews to the MDS staff.span /span5.span /spanPrepare face sheets for new admissions using the Resident Care System.6.span /spanCommunicate with Nursing and Environmental Services regarding new admissions.7.span /spanPrepare and send daily Admit/Discharge email.8.span /spanAssist with conducting facility tours.9.span /spanSupervise the Admitting Nurse Admission Specialist.10.span /spanKnowledge of census in excel, ability to assist with updating census when needed.11.span /spanResponsible for maintaining current knowledge regarding Medicare, Medicaid, and Third Party Payers.12.span /spanCoordinates Long Term Care Placements by reviewing applications and communicating with families regarding long term bed availability.13.span /spanExhibits excellent customer service skills and behaviors toward internal and external customers and co-workers.14.span /spanAdheres to department customer service, service excellence, and performance standards. 15.span /spanDemonstrates personal commitment to the Organization's core values through active involvement in the performance improvement process.16.span /spanDemonstrates safe work practices and attitudes, follows safety rules, works to prevent unsafe conditions and behaviors, and participates in organizational and departmental safety programs.STANDARD QUALIFICATIONSTo perform this job successfully, an individual must be able to perform each essential duty satisfactorily.span /spanThe requirements listed below are representative of the knowledge, skill and/or ability required.span /spanReasonable accommodations may be made to enable individuals with disabilities to perform the essential duties.A.span/spanEducation / Training:span/spanspan/spanA level of knowledge normally acquired through the successful completion of a Licensed Practical Nurse Program.B.span/spanWork Experience:span/spanspan/spanOne year of work experience as Licensed Practical Nurse in a long term care setting.C.span /spanCertification, Licensure, Registration: Current license to practice as a Licensed Practical Nurse in the State of Michigan. div /div /divBR/Beaumont Health is an equal opportunity employer.
