Kforce Director of Population Health in Detroit, Michigan

Kforce has a client in Detroit, MI that is looking for a Director of Population Health. The Director of Population Health is responsible for managing the clinical aspects of programs, initiatives, and contracts as directed.Essential Duties: * Is responsible for the design and implementation of business processes * Has direct reports * Has leadership responsibilities for subordinate leaders and staff * Serves as liaison with external organizations for implementing and monitoring of assigned healthcare programs * Oversees and participates in the evaluation of the operations of benefits, programs, and projects related to population health management * Informs program development, goals, objectives, and budget in collaboration with leadership * Ensures program performance to agreed-upon contract terms and execute steps for corrective actions when necessary * Analyzes data to assess clinical performance, including the creation and oversight of member-and-program-specific dashboards; Review and take action on reports to identify opportunities for improvement * Prepares clear and concise oral and written reports on assigned projects * Bachelor's degree in Nursing or other health related field * Registered Nurse with an unrestricted license * Master's degree in healthcare administration or related field preferred * Appropriate clinical certification such as CCM, CCP, ACM, or ABQUARP preferred * 5 years of experience managing clinical and vendor-supported programs and healthcare solutions * 5 years of experience managing projects in a health care organization including health plan, health system or hospital, physician group, etc. * Demonstrated responsibility for organizing key stakeholders and preparing written and verbal reports for executive leadership * Experience in health care quality and process improvement including project planning, measurement, and evaluation of quality or cost outcomes * Experience in population health management including quality, cost, and member experience improvements among a retiree population with a high disease burden preferred * Experience with Medicare benefits, programs, and regulations Kforce is an Equal Opportunity/Affirmative Action Employer.
