Munson Medical Center Health Care Educator in Cadillac, Michigan
Licensure: Graduate of an accredited school of nursing and currently licensed or eligible for licensure in the State of Michigan.
Certification: Certification as Diabetes Educator (CDE) from the National Certification Board for Diabetes Educators (NCBDE) preferred
Education: -Bachelor's degree in Nursing, or related field preferred.-Comprehensive knowledge of nursing practice and theory required -Knowledge of adult and child learning theories and current concepts of patient education desired -Will obtain CDE as soon as certification requirements are met -Demonstrates ability in using the computer with Windows format preferred.
Experience: -Minimum 2 years nursing experience in an acute care setting required-Previous 1-year experience in diabetes education desired. -Experience in learning needs assessment and program planning, development, implementation and evaluation desired. -Expresses enthusiasm for implementing and promoting patient education as an integral part of nursing practice.
ORGANIZATION:Reports to the Manager of Diabetes Education
Receives directions from the Diabetes Education Program Coordinator.
Has constant working contact with outpatients, hospital staff, managers, and general public.
Supports the Mission, Vision and Values of Munson Healthcare.

Promotes personal and patient safety.
Uses effective customer service/interpersonal skills at all times.
Due to travel needs of the position a current driver's license is required.
Applies the principals of the nursing process to diabetes education
Assess the learning/emotional needs of outpatients with diabetes and their families
Communicates learning/emotional needs to appropriate health care professionals and physician.
Plans appropriate diabetes education utilizing a multidisciplinary approach
Establishes with the patient/family specific goals to meet learning needs.
Implements teaching interventions with patient/family
Documents and evaluates teaching and patient/family response to diabetes education
Revises teaching plan as needed
Provides follow-up after discharge with outpatients and communicates relevant information to Healthcare Provider.
Acts as a resource professional on diabetes and its management to community members, hospital personnel, health care professionals and nursing students.
Collaborates with hospital personnel to develop diabetes education programs.
Establishes and maintains a current diabetes reference library.
Networks with local, state and national diabetes organizations.
Complies with continuing education requirements mandated by MDHHS & AADE for Outpatient Diabetes Education Program certification/ accreditation.
Acquires and maintains current knowledge and competency in nursing practice and complies with Michigan re-licensure requirements.
Acquires and shares knowledge of new research on diabetes education and management.
Participates in appropriate hospital committees.
Makes decisions and acts on behalf of patients in an ethical manner. Maintains confidentiality of patient information.
